This amendments only, not final lease.

certified or corporate check direct deposit to Community

Bank with receipt to bank account: - acct 2610249308.

Ii will provide routing number upon request.

There is a $5.00 processing fee for credit card or paypal.

Tenant must have proof of renters insurance. Must have

4504 south creek LLC as additional insured.

Lease: 12 months - Preferred. -- all included in rent..

TV set, all electric, WIFI / spectrum service set up.

Garbage pickup, lawn maintenance, street snow removal

by town. Water and Sewer also included.

Driveway snow removal is not included – tenant

responsibility.

Beds – air mattress included. Any and all linens not

included.

Policy requires to give landlord - 4504 South creek LLC

notice of termination and cause - One month (30 days)

Each month- Tenant will invite landlord - 4504 south creek LLC for a conference - if tenant fails to arrange it, then it is grounds for termination of the lease.

Upon entry and again at exit: a checklist of status of contents as well as status of floors, walls, counter tops, mirrors, rugs - etc. is reviewed and signed / dated by tenant.

One Set of keys provided:

for kitchen door and garage door.

Keys must be returned upon exit. No additional keys shall be made - if there are copies, they shall be re-surrendered at termination.

July 4th activities – cottage picnic / fireworks – landlord needs facility for vacation.

Columbus Day weekend 2019 / 2020– cottage not available. For tenant stay

Upon agreement- Landlord will provide substitute housing for that time period.

Cottage is set for window, floor, and ac/ heat pump installation Fall 2019 – These will occur depending on contractors scheduling. Upon agreement – Tenant should be able to reside while these updates are being completed.
